[cad-linux] BMP extractor

I have the first cut of it working but the bitmaps come out kind of small; 
actually they come out the same size that you see in the file open preview 
dialog of ICAD.  They look fine in the preview dialog, but appear really tiny 
when you open them in a paint program.  Is this going to acceptable for your 
purposes?  

If you think so I'll put the finishing touches on it and send it along.  I 
have a few segmentation faults to fix (I know where they are and how to fix 
them, but I need to call it a day since I have to drive across New England 
tomorrow).
