The disemu is a better solution to me than the BIOS emulator, it is not as fast 
as MemDisk and doesn't allow the read-write ability for the emulated images, 
but the BIOS emulator have the same limitations plus many compatibility issues.
For the millennium disk check (The Real DOS-Mode Patch for Windows Millennium 
http://www.geocities.com/mfd4life_2000/ , I didn't try it but it could benefit 
the WinMe disk).

Is /dev/ram0 a valid device in the Kernel, I think it needs a mount command or 
other initialize command before being valid, have you tried /dev/cd0, it worked 
well with me.

I need a minimalist distro to include in my 911 CD project as an alternate 
recovery method, what is the one prefered ??
Also I need the help from a Linux expert to edit post.rc in an initrd.gz file 
(specifically the Windows 2000 Password reset disk).

For MacOS X there is a free project to do such bootable CD, I don't remember 
its URL now.
