I usually used the palm of my hand straight up and down on the spine and if 
there's a little dip in the middle of the spine, I will put my first finger in 
that dip and hold the spine down.

  I usually use my index finger and thumb of my right hand to press on the 
spine or the side of the spine and then after I click scan then I take that 
hand back to the book and use my palm of my left hand to press on the left side 
of the book to keep those pages flat against the glass. Especially with 
paperbacks sometimes the edges want to stick up in the air.
  It has to be adjusted for various books though depending upon how worn they 
are or how new they are, if they're warped from water damage or if parts of the 
book are tighter than the rest of the book. I sometimes use my right forearm to 
hold down the pages on the right side of the book in addition to using my right 
index finger and thumb to push on the spine.
