Mike,
Did the book eventually work, or did you have to release it again? This
problem wasn't caused by anything you did, it happens to everybody once in a
while. The usual solution is to open the file in Word, probably other
standard word processors would work too, make some minor change like hitting
space and then backspace, and save with ctrl-s. After that it should work.
