It's not that we should remove bibliographies or indices; in fact, they are
quite desirable as are footnotes. It's just that if the scan mangled them we
can still submit the book and have it accepted. So if the footnotes are
there and reasonably decipherable, by all means leave them in. But if
they're a total disaster and wouldn't be usable, you can take them out if
you want. It would be a good idea to put a note in the long synopsis if
supplementary parts of a book aren't included because of scanner issues or
for whatever other reason.
