I have encountered a similar problem with books I've validated and submitted. I actually had a book rejected and then approved a few days later. I have no idea what went on there.

There is something else that bothers me about these bounce backs.  The way
this is done,  unless one happens to check to see if their work bounces
back, one would be clueless that this has occurred.
Might it be possible that the system be set up so that any action taken by
someone to
a book generate an automated email both to the submitter and the latest
validator?
Right now, as I see it, the weak link in the BookShare system is the
validation pool where books often books sit seemingly forever; and part of
the reason may include just the kind of situation you stumbled upon.
I recently stumbled upon a validation of mine that had a similar fate,
resubmitted it from my hd, including the requested info.
So far, while it hasn't yet made the collection, it hasn't bounced back
either so I am keeping my fingers crossed that all is well with this book.
