The other option is that someone didn't check the being scanned list and scanned a book that another volunteer was down for. If they did notify the list that the book could be removed, at least the person down to scan would know not to scan it or to abandon their work.

A person may, for example, decide to not scan a book they
previously thought they would scan. Or a volunteer who said they
were going to scan a book leaves the volunteer community, or
becomes ill or has to put their energy elsewhere.  Or someone
says they want to scan a book, and can't find a copy anywhere.
Those are some real-life examples of other ways a book can get
removed from the being scanned list. smile.
