Well, I'm going to be submitting a book in a day or two which has the chapter headings protected with one return above the chapter number. The page numbers are at the bottom of the page. I suppose it could be considered a test book? I'll give info on the book when I submit it. It is also the one about which I have been raising copyright questions. I am reading the book before submitting.
