Hi,

I used Windows Vista Home premium and my screen reader was NVDA.

The check boxes appeared as checkboxes and the edit fields appeared as edit
fields.

Oh, yes, I loved being able to write what sentences I wanted. That's the best
part of the survey. I hate those surveys that say "on a scale of 1 to 5, how
would you rate ..." I can never feel the survey is asking what I can answer.
Regards, Kim Friedman.

Me, too. Thanks for having the spaces where writing an answer was
required large enough to contain the comments

I just completed the survey with no difficulty at all.

          Hi, Scott,

I finally got the survey to work, but had to put a longer answer in
question one before I could move to the next page. Also, on Section three, the
survey threw me back to the beginning and I had to complete section three again
even though it was previously completed.

            Hi, Scott,

I have attempted to take the survey as you requested. For my first
three answers in Section 1, I entered an answer into each of the edit boxes.
When I pressed enter on the "next" button I could not proceed, because my
answers were not recognized.

On the answer to Question one, I entered the information in the
edit box under "other"; there was no way to check other.

I have taken a number of online surveys so know that I am doing
things correctly.

            Lori C.
