Hi, Scott,

I have attempted to take the survey as you requested. For my first three answers in Section 1, I entered an answer into each of the edit boxes. When I pressed enter on the "next" button I could not proceed, because my answers were not recognized.

On the answer to Question one, I entered the information in the edit box under "other"; there was no way to check other.

I have taken a number of online surveys so know that I am doing things correctly.
