I haven't finished reading all the posts, but yes--I
do move the page numbers to the top of the page--but
I've begun doing that only recently, after Lissi
posted that that makes it easier for blind people to
navigate. Thus the many books I've validated over the
years will have the page numbers either at top or
bottom depending on where they were in the book.

And yes, I do remove headers manually, but I must say
that submitters seem to be doing that nicely these
days, because I haven't had to do it very much lately.
And I do put in chapter titles if they are missing.

If you don't have the book, if I or another sighted
person can get it we can give you a list of the
chapter titles and the page they belong on. It might
be possible to get the titles from B&N. I found a
table of contents there for a book someone--Lora
perhaps?-- was doing and sent them to her. She had the
numbers but the titles were garbled or nonexistent in
the table of contents.
