Sounds kind of like a textbook. Sometimes they have a couple pages of acknowledgements and so on before the actual title page, but if you get to the table of contents before finding anything, then I'd say the cover page just isn't there. If the copyright information is there, then with the full title and author names previously provided by Cindy, you have the basics for it to be accepted.
