Yes, the asterisk in place of an apostrophe in
possessives is a scanning error and shold be
corrected.

Yes, there can be several titles pages; sometimes the
pages have just the title, sometimes title and author.
Usually there is a blank page or another page, like a
list of the author's other books, or the copyright
page--there should only be one copyright page. The
duplication can be annoying, but I always leave those
pages and the blank pages between, in. sometimes
there's a page with an illustration.

If you have the 2001 edition (forward by Billy Graham)
I can get the book for you if you want and
doublecheck, but I really don't think it's necessary,
unless you might have pages that need rescanning.
