This sounds like the form I filled out last year. I went to the homepage
and clicked on individual membership. It took me to this page,
http://www.bookshare.org/signUpType
which appears to take you to the right spot to begin the sign up process.
I wasn't logged in, perhaps this link doesn't appear if you're logged in.

It's been a while, but I don't remember filling out a form. I had
to send in proof of disability which in my case was my NLS
membership. They checked it out and that was it.
