Gerald answered your question about headers; but I'd like to bring
something else to your attention.
Be sure that each new chapter heading is protected from being eliminated
by the automated stripper.
I do this by placing a couple of dashes (--) or a series of asterisks (**)
above the line on which the chapter name, number or word chapter appears.
Sometimes, I do this by placing the word chapter on one line and its
number on the line below.
In short, attempt to keep this info 4 or 5 lines at a minimum from the
top of the page so that it remains there.
