Well, my understanding is that the heading text needs to be 4 points larger than the rest of the text, so that it will be recognized by the conversion tool as a heading. If you're going by the philosophy that the level 1 heading should be reserved for the book title, I would guess then, that you'd need to make the book title even larger.
