I always put a blank space at the top of the page and then the page number, then another blank space and then the chapter heading. That is I treat it just like any other page. Supposedly, the Bookshare tools will move page numbers from the bottom of the page to the top, but I don't know how well this works, so when most page numbers appear at the top of the page but at the bottom on the first page of a chapter I feel more comfortable moving it to the top myself.
