Kelly,  headers are removed automatically only if the Bookshare system 
recognize them as such,  otherwise they stay in the final copy.
This means,  that poor quality headers may end up being evaluated in the 
final quality rating.
The safest thing is to remove them prior to the final submission and leave 
in purely page numbers.

Hi Robert,
Please don't base your quality rating on how well the headers scanned. 
They
are removed anyway, so this is not relevant to the final product. Base the
rating solely on the quality of the body text of the book.
