Probably a silly question but do you need something more than PDF Reader
to fill in an electronic PDF form?  If so, perhaps that's why it never
caught on.

There are such things as interactive PDF documents (I think that is the
correct term), but I have never seen one in the wild.  These allow you t
fill them out, I once had a demonstration document that does this and it
even worked with Jaws.  However PDF documents consist almost entirely of
non-editable documents principly designed to be downloaded, printed and
filled out by hand.  The electronically fillable PDF has never caught on
despite being potentially in existence for over 5 years.
