There are two sides to the Last.FM service the player and the website.
The official player is not accessible (not easily anyway). So even if you
download the software you will be wasting your time.
I have developed a player for the Last.FM service as part of the SpeakOn
system which is available for free at:

If you insist on using your screen reader with the player read no further,
SpeakOn is self voicing and while I tried to maintain Windows keystrokes
compatibility it works in a different way; for a start there is no visual
interface and you will have to put a bit of effort to learn the system. Once
you know how to use it, it is easy; everything works the same way across all
9 types of text and audio media that it supports.
With SpeakOn you can listen to all sorts of music and interact with the
server; getting information, skip tracks mark tracks that you love, ban
tracks and play music similar to your favourite artists. Using the player,
I have little need to go to the website so I go to it rarely - it is useful
for some people especially if you want to know about artists, buy music, and
find information about live performances, there is also a significant social
aspect to the website that some people are interested in.
I find the site accessible, I have spoken recently to a number of SpeakOn
users who use the site frequently and they tell me the same; it is perfectly
accessible. Moreover, one of these people has approached Last.FM a number
of times for accessibility improvements to their website and they always
responded. By the way these people happen to use either JAWS or
Windows-Eyes.

I couldn't' even complete their installation wizard for th edesktop
software.
It's design was in a word horrendous.
