As you have found, I doubt that many (any?) of the printer driver PDF
creation tools can do this automatically.
Back in 2006 I did some testing of various cheap PDF creators for a
client - the results of my findings are here:
http://www.cybertext.com.au/tips_acrobat.htm This doc is a little dated
now, but I suspect the overall results still hold.

Thanks for your suggestion about Open Office.
I particularly need all the cross-references (not just the TOC) to
become hyperlinks in the PDF file. Do you know if this happens? Can Open
Office co-exist with Word 2003?

Thanks for your suggestion. I had a look at Bullzip. It sets itself up
as a printer. My previous experience has been that making PDFs on a
pretend printer never produces the hyperlinks I need, only plain text.
