Thanks for your suggestion about Open Office. 

I particularly need all the cross-references (not just the TOC) to become
hyperlinks in the PDF file. Do you know if this happens? Can Open Office
co-exist with Word 2003?

Thanks for your suggestion. I had a look at Bullzip. It sets itself up as a
printer. My previous experience has been that making PDFs on a pretend
printer never produces the hyperlinks I need, only plain text.
