possibly related to the jason tab? if you clear the styles after you create the 
toc, what happens? (ctrl-space and ctrl-q)

the heading style associated with the level 3 toc style doesn't have it's own 
leader, does it? - that could be confusing things.

Been there, done that already - several times. Still get leaders on TOC3 
[Heading 3] and not on TOC1 [Heading 1] and TOC2 [Heading 2].

Undocumented feature of Word 2003?
