I generally use Ctrl V, but also tested the Right-click menu and toolbar
button. And I checked the short cut keys were valid in the Customize
menu.

I don't use shared Normal or other templates, so no-one else would be
changing my customizations in those.

select Paste from the Edit menu with the mouse.

If you use shortcut keys, it's possible that someone
has used the same keystrokes for a macro. 

That's what happens when people who don't use shortcut
keys create macros. I had one where someone used Ctrl
S to send the document via email, and another that had
font formatting linked to Alt O. There go the Save and
all Format shortcuts.
