I like Reena's approach, especially for online text. (Except I'd just say
'box', or maybe 'field', depending on the audience, not 'text box'.)

If you put the results of a step on a separate line, I wouldn't use a
different font. The fact that the information is on a separate, indented
line is enough of a cue for the reader to know what it is.

In the new version of the Microsoft Manual of Style, Microsoft has changed
the way it suggests steps are written. I don't have a copy of it yet; has
anyone got one?

Yes, screen is normally used for Display. I prefer a window or a form. E.g.

1. Click OK to open the <name of the form> form.
2. In the Name text box, enter your name.
