I don't think I have seen any previous message from you on this issue. But what I meant about a separate graphics file of course means that you must have place holders.

Hmmm, Rafi.  What's wrong with my suggestion of picture placeholders?

But Bede got it right. Who on earth puts Word files on-line? Even if they are converted to PDF first, there could be lots of individual files, with an
HTML TOC.  Even though it is a nuisance to generate a TOC from multiple
files, it is a trivial job. So the real question does become "Why use Word
as the publishing tool?".
