what I meant about a separate graphics file of course means that you must
have place holders.

But Bede got it right. Who on earth puts Word files on-line? Even if
they
are converted to PDF first, there could be lots of individual files, with
an
HTML TOC. Even though it is a nuisance to generate a TOC from multiple
files, it is a trivial job. So the real question does become "Why use
Word
as the publishing tool?".
