I think you will find the html help is set up within a Frameset.
Open the Index.htm page in notepad and I bet you see a few lines of code 
with frameset and frame tags...
Thats why the TOC won't open when you open the specific page.

Add a toolbar or a button to the page linked in the EMail that will send 
you back to the main index (the frameset); (a simple quick and dirty fix).

You can probably add a javasript to determine whether the opening page was 
linked through the frameset or linked from an email source. The javascript 
could then direct the toolbar/button to be loaded or not. 

You could probably write a js to open the frameset too, if the page is 
opened by EMail link or other such. I don't know how Robhelp would 
cooperate with that.
