Ok I've worked this one out.
Here's a quick and dirty rundown.
1. Open your PDF.
2. Position the view where you want the message to appear.
3. Click Tools > Advanced Editing > Text field tool.
4. Draw a rectangle where you want the message to appear.
5. Click the General tab and give the control a name.
6. Change the Form Field drop down to Visible but doesn't print.
7. Click on the Options tab.
8. In the default value field, enter the text you want to appear in the
message.
9. Under the Appearance tab you can change the font size etc.

I think the form Daryl mentioned is created using Adobe Designer. Very
cool software. Although the age of the form is curious. Are you sure of
the Adobe product (Distiller 4.05) listed Daryl? It seems a little
early. I think you can design the forms to open in earlier versions of
Acrobat but lose some functions. So maybe that is what has happened
there...
The red note looks like and is probably some java layered within the
form. When I opened it the last line was cut off along the bottom in
line with the next function on the form. Layer overlap is a classic Java
glitch.
Now if you have Adobe CS2 handy you will find the latest version of Form
Designer in there, though you might need a Java guru to sort it out.
Otherwise it looks like a pop up note as suggested by others!

The Javascript option may suit, or maybe this...
Have a look at the form used for land transfers in NSW. Go to
http://www.lands.nsw.gov.au/FormsAndFees/Dealings/Interactive/default.ht
m.
The Transfers link is a PDF of the transfer form (which has a really
long URL). Have a play with the radio buttons in the lower half of the
form, which are cool (or seem so to me).
But importantly for your purposes, observe the PLEASE NOTE!!! message in
red at the top of the form. This message does not print. Neither do the
'Change Execution' buttons which appear in the lower half of the form
when you hit the radio buttons.
Unfortunately I don't know how they achieve it, but perhaps it's a start
for you. As the Pure Mathematician said, 'there _is_ a solution!' I
first encountered this form last year, before Acrobat 7. In fact,
looking at the properties, I see that it was made with Distiller 4.05
and can be opened by Acrobat 3 and later, so it's probably quite simple
stuff, if you can find it in the book or the help, or perhaps just by
playing with the form in Acrobat.
If you want to be cheeky, the properties even tell you the name of the
author, so you could ring the Department and ask for that person!
