Any time you think it's going to work at last, it won't.
Look for use of [SEQ] fields or whatever.   I'll see if I can dig up 
references, but I'm sure that there'll stuff in the archives on all
this, and I'd be surprised if the old S.H. didn't post some stuff on this.

For years I kept running a macro I had that reset the numbered list start 
number, but it was still a pain in the bum if you had ten or more small 
numbered lists and then reopened the document to find all list items numbered 
sequentially from 1 to 50 and beyond...

There are some macros around for free which did the setting and resetting of 
SEQ fields. ... ahha !  Check out for starters,
