For meetings with SMEs that I know are going to ramble I take my Powerbook in 
with me and just use MS Word.  It has 
a Notepad layout which when selected provides built in voice recording.  That 
way it records the meeting whilst I type 
particular points directly into Word.

After the meeting I can then review/rearrange my notes while replaying the 
conversation.  All are stored as the one file so 
makes it easy for filing when I've completed a few sessions.

Only rule though is always ask first before recording.  Never yet been turned 
down, but always ask.
