Christine,
The old format for letters you describe reminds me of when I attended typing
classes at Sydney Tech College in 1983.
We used a book that was probably foolscap size, and bound across the top so
we could stand it beside our typewriters and flip the pages up and over.
It had a few letter layouts that were described in great detail.
The title was something like "learn to touch type in two tedious terms".
