I've never seen it happen either.  But with the vague memory that I 
had, I couldn't remember which help format or authoring tool was 
referred to - so I thought I'd throw it in and see if it was actually a 
problem anywhere, since I've only used RoboHelp and Windows 
HTML Help so far.

Yes, it is a peculiar article.  Hence my suspicion that it was written 
about an ollllllld version of Robohelp, maybe when people were still 
writing help for Windows 3.1 or DOS environments.  But on further 
investigation, it appears to be part of a relatively current (last year) 
subject called Technical Editing at the University of Alabama at 
Huntsville... hrmmm.  Curious.
