Thanks for making the effort, Howard.
Also thanks to Terry Dowling and Neil Maloney, who have
been assisting off-list and to Steve Hudson, who gave
some typically sage words of wisdom over the phone.
(None of which can be repeated here.)
One of the more interesting things that has come up, thanks
to Neil's efforts, is that there is a recognized registry
problem that can cause this after applying a Word 2002
Service Pack. (Ah! Dear old Microsloth, aren't the Gates
Brothers just marvellous!)
See "How the behavior (sic) of the Word fields changes
after you install the Word update" on the Microsloth US
(hmm, in the Australian Army Cadets, I remember that US
used to always stand for "unserviceable", is this a clue?)

More later, when I know more.
Hey, I even resorted to calling the Help Desk, at my
customer's site where the problem originated.
Why not share the pain, I say!
So in terms of anything else you can try, please try
accepting my thanks for your efforts and relax until I
provide a further update.
...and, says he (slurping, John Hurt style, from beneath a
crumpled dark Hessian sack): "I amth noth a Lethargic Man
(gasp, slurp, gasp) I amth a Techthnicle Communicathor!"
Cheers,
Michael

This probably will be of no help whatever to you, but at least I tried ... .
(After all, you did help me with my Acrobat problems.) Oh yes, and I should
add that I certainly couldn't claim to be a "Word 2002 guru", but that
didn't stop me.
I attempted to recreate your problem in Word 2003, but have been unable to
reproduce the symptoms. Which means, I suppose, either that your problem
derives from a bug in Word 2002 that has been fixed in Word 2003 or that my
reconstruction of your scenario was inaccurate.
This is what I did. I created a template containing a field
{INCLUDEPICTURE "C:\\Documents and Settings\\All
Users\\Desktop\\Howard\\web\\H Site\\images\\chair3.gif" \d \* MERGEFORMAT
}
and saved it. (Don't ask why I chose a picture of a chair for my sample -
maybe I was subconsciously thinking of the Lethargic Man.) I exited from
Word, then restarted it and created a new document based on the template
just created.
And it looked just fine. When I toggled field codes, there was the code,
unchanged, just as above. I tried it with the link locked and unlocked -
made no difference.
Is there anything else I should try?
Howard
