Ok. I've worked out what the problem was. I needed the Acrobat 5.0.5
patch in order for Acrobat to work properly with Windows XP/Word 2002.

Hi Bridget,
  1.. Select the Office tab
  2.. Move to the Word Feature region
  3.. Select Cross-References & TOC Links
I would also:
  1..  Move to the General region
  2..  Select Convert Document Info and Convert Internet Links

Do NOT select Embed Tags in PDF from the Document tag region.
