atw: In-house disclaimer

Folks
Some documents in this project will include screen shots that show
names, addresses and phone numbers. I have removed the information
about known real people by faking shots and bit editing where necessary,
but I want to add a bit of protection by adding a notice in the front
matter.
This is what my notice says:
This document contains illustrations that show names, addresses, phone
numbers and other identifying information about ficticious persons
invented for the purposes of illustration. Any reference or implied
reference to any real person, living or dead, is accidental and
regretted.
Does anyone have any experience in this kind of disclaimer?
Do you have any suggestions for a better wording?
