atw: Briefing Document from Clients

Hi All,

I've been asked to come up with a briefing document that clients fill in 
whenever they want some technical writing done.  A bit like a proposal.  I have 
some ideas, but am wondering if any of you work with such a template/document 
already and would be willing to share?  Or some advice on what you must get out 
of the clients before starting.  I don't want to reinvent the wheel.
