atw: A couple of problems with PDFs

I am creating PDF files from Word documents using Acrobat v 7.
I set hyperlinks in the Word document then use the conversion settings in
Acrobat to make the links active in the PDF file.
So far so good, but there are a few issues:
1) Links from the PDF to web pages or other documents are fine ... until you
try to come back.
The back button returns to either the end or the start of the PDF document
(I'm yet to work out why some come back to the end while others come back to
the start)
Obviously, I'd like my users to be able to return to the page they were on in
the PDF.
The problem would be solved if I could insert a 'target="blank"' switch in the
hyperlink itself (to open the destination file in a new window) Can you do that
in Word?
2) Recently I've been finding that some links from Word files simply aren't
converting at all.
Anyone shed light?
