Do you already have your CD's? If so, and your looking for a small 
quantity, your best bet might be to go to someone who does digital 
printing. The specs are up to your designer to get right (I can help 
you - after July 1!). If you need MANY made, your best bet is a place 
like psprint.com - following the links for their "club cards" or 
"flyers". I just got 5000 full color front and back 4x6 postcards for 
around 300 (incl shipping!)

If you HAVEN'T got your CDs made, most replication house offer a 
package deal including inserts. You should be able to get around 1000 
- shrink wrapped and ready to go - for around 1200-1300.

Or you can do what we're going to do for our next CD. We're going to 
use a combination of http://www.musicbizacademy.com/mixdown.htm for 
web sales and 
http://www.diskfaktory.com/secure/enter/mf/default_011703.asp for 
disks to sell at gigs and to send to reviewers. If sales justify it, 
then we'll see about a 1000 disc run, but we've got boxes and boxes 
of our first CD still sitting around!
