It was the first ever X rating for violence IIRC, but to be honest, it's violence is quite tame compared with a lot of todays R movies. It probably wouldn't rate an NC-17 today. The biggest difference between CO's violence and something like Pulp Fiction might be that the "droogs" are doing all of their misdeeds strictly for fun... and, Alex, the worst of the lot is portrayed as cool and likeable. He's the HERO for goshsakes!
BTW - In Droog-speak, moloko means milk. Moloko plus is drugged milk. Synthemesc and Drencom are tow of the drugs the put n the milk. These details aren't readily apparent in the film. They're treated sort of matter-of-factly.
Clockwork Orange has infiltrated much of contemporary culture - some examples: Bowie sings of his "droogies" in Suffragette City, "ultra-violence" has become a buzzword, Bart was dressed up as Alex in one of the Simpson's Halloween specials, an 80s band was called Heaven 17.
If you do see the film, you'll never hear Singing in the Rain or the William Tell Overture the same way again (or the 9th symphony for that matter!)
