[amayausers] order form template

Hi!
I have a question. I am doing a "spirit wear" order for an elementary
school and they asked me to create an order form that the parents can use to
order the t's and sweatshirts from.
I rarely do business with schools and do not have a form available. I have
tried searching for a form template that would work, but I didn't have much
luck.
I was wondering if anyone may have a template for an order form that you
could send me? (I am going gray over this) :-(
