If this is a woven fabric I would look at 2 layers  of tearaway.

I'm putting a 11" wide name on the back of a dress/work button down shirt.
Its above the yoke were it is double thick. I'm using two layers of 2oz cut away on the left chest, but, that seems like it would be real heavy on the back.
