[amayausers] Stitch problem

I've run into something new, and it's creating a big problem! I'm doing 
Group Names on polo shirts. I'm using cut away and Solvy and Melco Micro 
Block at .5 inches in black thread. Two of the three names I've done so far 
are pulling out.

My questions are:

1. Why is this happening?

2. How do I fix it (the worst one is still hooped. & I'm hoping I stopped 
the other one with Fray Check, but an afraid to continue until I solve this 
problem.

I need to solve this tonight so I can ship the shirts tonight or tomorrow 
morning before 8:00 am.
