That's a birdsnest.  You need to clear out your bobbin area and hook.  You most 
likely have thread caught up in there.

Other causes of holes - too dense a design - needles that need to be changed.

Can you sew another design on one of the ruined shirts to see if it's design 
specific after you clean out the bobbin and hook area?
