Honestly, I eyeball.
I debated for quite some time prior to opening my business, I even posted the 
same question a while back.
After practicing and creating a template on my counter top, I have gotten 
extremely accurate.
I just did a  run of polos and wind shirts for a golf course and you could not 
see any difference in placement on the shirts.
