You'd have to use your 5" x 4" but only apply the hat part way on the  frame. 
Adjust your placement of the design it's in the proper place. Of course  
you'll remember to turn the design upside down. :-) Oh, and watch as the design 
 
gets close to the arm/needle plate area. If the hat pulls down the frame even  
slightly, it can mess up the registration--you want to make sure the frame 
stays  above the plate area (I have held the garment so there is no weight and 
even  lifted the frame "slightly" to make sure it clears). Hope this helps.

I've never done one of these but have you tried heating the  hat a bit
(the  
area to be embroidered) and then use a fast frame?  How large a design?
Is it  
possible to put the design on a cloth  strip that goes around the base? 
Hopefully  someone with experience  will help out.
