You mentioned this is your first time embroidering dress shirts. Usually issues 
like this are related to the hooping. Make sure the material is pulled taught 
in the hoop (Don't stretch the fabric.) Also make sure you are using underlay 
in the letters to tack down the backing to the shirt before the lettering gets 
sewn on. I usually use edge walk at 60% on lettering. Also make sure letters 
are at least .3" tall
