John,

I do this type of lettering all the time for athletics.  I use Dalco because
I have their stockstitch program which works with their stock letters, #'s
and symbols.  I have also had them create custom applique' for me for
softball uniforms.  Not sure how reasonable they are but you can also try
Stahls, they do the same thing.
