Steve, George's detailed analysis seems to me to be on the
right track.  On 
one occasion I left Window-eyes running in the early stages
and only quit it 
when SA2Go reported that it had detected another screen
reader.  
During the download of the small exe file I followed the
progress and 
noticed that another 100 k or so was still to arrive when
the prompt 
instructed me to press alt and R.   
The other point is that when it comes to the screen on which
you type 
account number and pin I have sometimes tended to silence it
with the Ctrl 
key and then it will not accept the input data.
  
I am convinced that timing is at the root of the apparent
inconsistency.
