I don't personally think the voice is that clear on the iPhone.  Have you tried 
the American or Austraillian variants?  The latter is pitched slightly 
differently and any twang in the accent might actually be useful information.  
The answer is that there are hearing loops available to work with the iPhone.  
I am across a page with these on without intending to look for them.

What about people who have a nearing impairment and find the Iphone voice
difficult to hear, as I do as it is a female voice and I understand that at
present you can't change the default voice that is the reason why I am
reluctant to go down that road at the moment as I can't hear it all that
well.
So please can people stop being sarcastic and answer the questions in a
civilised and helpful manner  or keep quiet

Dave,

Why can't people ask a quite legitimate question without it
being put down by a sarcastic comment?  What will people say
when Apple have killed off all the competition and there is
no choice out there?
