Mary,
Personally I prefer to have more than one Spyware program on my 
systems as they tend to detect somewhat different threats.  I currently 
use Spybotsd and Ad-aware Se Personal and find that whichever I run 
first the other usually finds something new. 
Neither has an ideal interface for working  with a screen reader, but I 
use both satisfactorily with Hal and I am sure with a few tips from a 
Window-eyes user you should be OK.
